Coverage depends on the procedure and your plan. The standard monofocal intraocular lens used in cataract surgery is covered by Medicare Part B and most commercial insurance. Premium intraocular lens upgrades, such as the Light Adjustable Lens, toric, and multifocal lenses, are not covered by insurance.
